You can sign up for our Healthy Smiles Plan to instantly spend less and get more at the dentist, all without having to deal with waiting periods, deductibles, yearly maximums, or claim forms. Adult Plan

502 /year

-or- $276 down/6 months of $46 payments ($552 in total)

Each additional adult is $400 yearly
OR $225 down/6 months of $37.50 payments ($450 in total)

Child Plan

400 /year

-or- $225 down/6 months of $37.50 payments ($450 in total)

Each additional child is $352 yearly
OR $201 down/6 months of $33.50 payments ($402 in total)

The Healthy Smiles Plan Includes:

  • 2 cleanings per year
  • 2 exams per year
  • 1 limited exam per year
  • X-rays whenever necessary
  • 2 fluoride treatments per year
  • 2 oral cancer screenings per year
  • 15% discount on all restorative care (not applicable for orthodontic/whitening procedures)

Terms & Conditions

  • Monthly payments will be processed with a credit/debit card on file on the 1st of each month. These payments go toward your plan, not any treatment balance.
  • The program is a discount plan, not a dental insurance plan. It can’t be used in conjunction with any other insurance plans.
  • The plan is ONLY good at Parma Family Dentistry. Therefore, if you are referred to a specialist, discount offers will not apply.
  • The plan is non-transferable.
  • Dues are non-refundable. No refunds will be given if the patient does not use their dental plan.
  • Preventive care must occur within the year of enrollment and cannot exceed the enrollment date.
  • Monthly payments are unable to be canceled. If payment is not made, discounts will be removed.
